The role of neuroimaging in the diagnosis of idiopathic Parkinson's disease is sometimes contested. There is a school of thought that in patients with classical presentations neuroimaging is generally non-contributory. Parkinsonism can be caused by structural deficits though which may be apparent on MRI. Cardiac sympathetic denervation on metaiodobenzylguanidine (MIBG) scintigraphy - not very useful? SPECT?

Idiopathic Parkinson's disease generally responds well to levodopa. Dopamine-induced dyskinesias are reported to be more common when managing idiopathic Parkinson's disease than in other parkinson plus syndromes and can be used to support diagnosis.
