Definition

Double vision with vertical image separation with resolution on covering either eye.

Taxotomy

Aetiology

Vertical binocular diplopia with pure horizontal image separation is usually the result of a cerebellar or brainstem lesion. Rotation of the lower image may be evident in trochlear nerve palsy.

Clinical features

Tilt of the lower image is suggestive of a trochlear nerve palsy. In trochlear nerve palsy, tilting of the head away from the side of the palsy may be used as a compensation measure.
