Aetiology

Various neurological symptoms have been associated with antibodies to contactin-associated protein-like 2 (Caspr2). Caspr2 is essential for positioning voltage-gated potassium channels.

There is overlap in the pathophysiology of disorder associated with CASPR2 and LGI1 antibodies.

Investigations


One case series reported normal CSF in 65% of cases (https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4970662/). In the same series 70% of people had a normal MRI while in 24% of people there was bilateral medial temporal lobe hyperintensity. EEG is abnormal in around 70% of cases and epileptiform in around 40% of those. EMG may demonstrate excitability.
